- RN home visits — start-of-care and follow-up visits, medication review, and plan-of-care documentation.
- Care-plan review — eligibility and needs documentation that meets New York State standards.
- Korean-speaking members — assessments and communication in Korean and English.
Who we work with
Licensed home care services agencies, certified home health agencies, and managed long-term care plans in New York City and the surrounding region. Services are provided on a contract basis through the PLLC.
